5 Why Template

The 5 why technique is a simple and effective method of performing a root cause analysis.  The approach is most often used with the cause and effect or fishbone diagram.  The 5 why's are performed by asking why five times and listening as shown in the picture above.  To learn more about the 5 why analysis visit our post on our blog page "Listen to the Gemba" entitled, Why oh Why?

Cycle Time Template

  Cycle time is the amount of time it takes to complete an individual operation or task from start to finish.  Although cycle time is an easy metric to measure using a template to automate details like averages, sums and keeping forms of data organized can be very helpful.

Tollgate Template

Six sigma and many other forms of project management use "tollgates"  to check the progress of projects in relation to their objectives.  When an assigned individual approves that the team has successfully completed one stage of the project, the team then can "pass" on to the next stage of the project or initiative.  If the team does not successfully complete a stage they can not "pass" on to the next phase.

Example:

  In six sigma the DMAIC methodology is used.  Team members may establish toll gates between the Define, Measure, Analyze, Improve and Control phases of the methodology.  At each of these points the team must receive approval before moving on.

Control Plan

  The control plan is a tool used in the control phase of the DMAIC method. The tool is used for documenting the functional elements of quality control that are to be implemented in order to ensure that quality standards are met for a particular product, process, project or service. The intent of the control plan is to formalize and document the system of control that will be utilized in such a way that everyone can understand, while minimizing and controlling variation in the process, product or service.

A3 Template
